What information should be included in a DDS amendment request?

The Directive 059 FAQ says amendment requests should identify whether data are being added, deleted, or changed.
It also says to include from/to codes and values and the required supporting details, such as dates, casing, cementing, completion dates, and interval depths.

Write amendment requests like a change log. AER should be able to see the old value, the new value, the affected UWI, and the reason for the change without guessing.
